Para-esophageal hiatal hernia

Diagnosis almost certain

Presentation

Post prandial fullness. Abnormal chest radiograph. Had cholecystectomy 20 years ago. No history of trauma.

Patient Data

Age: 75 years
Gender: Female

There is a large hiatal hernia with intrathoracic displacement of stomach, gastro-esophageal junction(GEJ), left colonic flexure as well as the corporeocaudal segment of the pancreas.

Bilateral parapelvic renal cysts.

Features of a para-oeophageal hernia (type 4).

Type 4 hiatal hernia is characterized by an intrathoracic herniation of the stomach (which is usually abnormally rotated) along with other organs (colonpancreas, spleen, and small bowel).
