There is a large hiatal hernia with intrathoracic displacement of stomach, gastro-esophageal junction(GEJ), left colonic flexure as well as the corporeocaudal segment of the pancreas.
Bilateral parapelvic renal cysts.

Type 4 hiatal hernia is characterized by an intrathoracic herniation of the stomach (which is usually abnormally rotated) along with other organs (colon, pancreas, spleen, and small bowel).
