47 year old male presents with intermittent chest pain

X-ray

CXR

Subtle left paravertebral mass behind the left atrial appendage
X-ray

AXR

The left thoracic paravertebral mass is better seen on the AXR.
CT

CT chest + C

Longitudinally oriented left paravertebral mass, not related to the neuroforamina.
MRI

MRI with contrast

Post-contrast imaging shows the typical avid arterial enhancement of a paraganglioma, confirmed by histology from resected specimen.

Case Discussion

The longitudinal orientation and intense arterial enhancement are characteristic of a paraganglioma.
