Medial knee soft tissue swelling
Patient Data
There is a horizontal oblique tear involving the posterior horn of the medial meniscus, reaching the inferior articular surface and extending into the meniscal body. This is associated with a large multiseptated parameniscal cyst which extends around the medial collateral ligament into the soft tissue. The cyst measures 5.5 x 4 x 1.8 cm in its maximum CC, AP and TS diameters.
Case Discussion
MRI findings are consistent with an oblique horizontal degenerative tear of the posterior horn and body of the medial meniscus associated with a large complex parameniscal cyst.
A parameniscal cyst is a well-defined fluid collection seen adjacent to a meniscus. They may show lobulations, internal septations and rarely bony erosions. It is usually associated with an underlying meniscal tear. Medial parameniscal cyst is slightly more common than lateral parameniscal cyst. Parameniscal cysts, especially medially, can be very large.
