A 130 x 90 x125 mm thin-walled cystic mass with homogenous internal fluid density at midline pelvis that extends superiorly to L3 level. No detectable septations or enhancing solid components. No ascites.
Both ovaries are identified separately from the mentioned cystic lesion.

Huge pelvic cystic lesion; most likely paraovarian.
Paraovarian cysts typically occur in women ages 20-40 years old, generally around the broad ligament, and are usually simple cysts.
