Lump in throat with mild pain

Thought to be a deep lobe of parotid mass on US - biopsy showed spindle cells thus MRI performed

Case Discussion

Large, lobulated, well defined mass in the right parapharyngeal space abutting the pharynx and deep lobe of the parotid but not arising from either. Patchy enhancement with central fluid consistent with cystic change or necrosis. Biopsy indicates either schwannoma or neurilemmoma. Major differential diagnosis is accessory salivary gland tumour, e.g. pleomorphic adenoma or metastatic disease to a lymph node from a head and neck cancer.
