Multiple bilateral air filled cystic spaces with characteristic peripheral and subpleural location.
The ones ≤10 mm in diameter are considered paraseptal emphysema, and the ones with diameter >10 mm are considered bullae.

65 years old smoker patient with shortness of breath. CT demonstrates bilateral pulmonary paraseptal emphysema as well as subpleural blebs/bullae.
