Parathyroid adenoma

Diagnosis almost certain

Presentation

Generalized bone pain and easy fatiguability. The parathyroid hormone level is 103 pg/mL (normal value 10 to 55 pg/mL).

Age: 40 years
Gender: Female

There is a small homogeneous soft tissue nodule posterior to the inferior pole of left thyroid lobe measuring 10 x 12 mm. The enhancement pattern is not identical to thyroid gland enhancement with decreased enhancement in the delayed phase.

Case Discussion

Parathyroid adenomas are the most common cause of primary hyperparathyroidism. The vast majority are solitary and juxtathyroid. CT is valuable for preoperative localization and identifying adenomas at ectopic sites.
