Patient Data

Age: 40 years
Gender: Male

Lobulated lesion in the right parotid gland demonstrates T2 hyperintensity, peripheral rim of T2 hypointensity, high ADC value (approximately 1900) and heterogeneous enhancement.

It is a solitary lesion.

No concerning perineural enhancement along the course of CN VII.

No periparotid lymph node enlargement in the field of view.

Case Discussion

Imaging features are typical for a pleomorphic adenoma of the parotid gland.

Histopathology:

Sections show a parotid gland containing lobulated, focally unencapsulated tumor. The tumor contains mixture of epithelial and stromal elements with a strong predominance of myxoid stroma. The epithelium comprises small nests, cords and tubular structures. The tumor measures 15 mm in maximum dimension and lies 0.5 mm from the nearest resection margin. There is no specimen disruption. There are no infiltrative or malignant epithelial or stromal elements identified.

CONCLUSION.

Right parotidectomy:
Pleomorphic adenoma (stroma rich); 15mm; narrowly excised.
