Presentation

Painless swelling of the right parotid region in HIV positive patient.

Patient Data

Age: 30 years
Gender: Male
mri

Well-defined cystic lesion of the right parotid gland eliciting a low signal on T1, high signal on T2 with no restricted diffusion. The postcontrast sequences show a thin rim of peripheral enhancement.

Normal appearance of the left parotid gland.

peripheral mucosal thickening of the maxillary and sphenoid sinuses with partial opacification of the ethmoid cells.

Case Discussion

MRI features of a well-defined cystic lesion of the right parotid gland with no restricted diffusion.

The patient went on to have complete resection of the lesion with a histopathological exam that confirms the diagnosis of benign lymphoepithelial cyst.

 It is relatively common in the HIV population, with 5% of patients eventually developing benign lymphoepithelial lesion.
