Partial anomalous pulmonary venous return

There is hypoplasia of the right lung and dextroposition of the heart.
Mediastinal shift to the right side and also right diaphragmatic elevation also noted.
Anomalous (scimitar) vein paralleling right heart border.
Constellation of findings are suggestive of partial anomalous pulmonary venous return Scimitar's syndrome.
Case Discussion
Hypogenetic Lung (Scimitar) Syndrome
- 4 features typical (although each not always present):
- hypoplasia of the lung with abnormal segmental or lobar anatomy
- hypoplasia of the ipsilateral pulmonary artery
- anomalous pulmonary venous return (scimitar vein)
- anomalous systemic arterial supply to lower lobe
Other features:
- almost always on the right sidem
- mediastinal shift toward the hypoplastic lung
- reduced size of ispilateral pulmonary artery
- anomalous (scimitar) vein usually paralleling right heart border
- congenital heart disease in 25% (e.g., atrial septal defect and patent ductus arteriousus)
Clinical symptoms: recurrent infection and dyspnea.
