Partial small bowel obstruction caused by adhesions

Case contributed by Dr Bruno Di Muzio

Patient presented to the emergency department with history of diffuse abdominal pain. Abdominal x-ray showed dilated loops of proximal small bowel. A CT study was requested.

Patient Data

Age: 65Y
Gender: Male

CT Abdomen and pelvis

CT shows dilated loops of proximal small bowel and an abrupt transition from dilated to collapsed bowel segments in more caudal images.

Case Discussion

The patient had a previous CT study one year earlier with the same aspect.  He also has two enteroclysis exams described as normal.

The only hint of the presence of adhesions that we can find on CT is this abrupt transition from dilated to collapsed bowel segment. The case was presumed to be an abdominal adhesion causing a partial small bowel obstruction.
