Diagnosis certain

Presentation

Right shoulder pain with limited abduction

Patient Data

Age: 45

The supraspinatus tendon is thickened displaying intermediate signal in T1 and fluid signal within its substance in T2 and STIR images. No retraction. Mild fluid is seen within the subacromial subdeltoid bursa. Associated hypertrophic osteoarthropathy of the acromioclavicular joint is seen obliterating the subacromial fat and impinging the supraspinatus tendon.

Subcoracoid bursitis.

Degenerative subchondral humeral head cyst

Case Discussion

Acromioclavicular osteoarthropathy causes narrowing of the subacromial space and hence impinge the supraspinatus tendon and this account for subacromial impingement.
