Age: 70 years
Gender: Male

Fusiform aneurysmal dilatation of the basilar artery, with heterogenous mainly hyperdense content. The right vertebral artery appears small in diameter.

Large aneurysm of the basilar artery, with partial contrast opacification in the left side of the aneurysm.

The aneurysm is causing significant mass-effect upon the brainstem.

The right vertebral artery appears small in diameter but opacified with contrast.

Case Discussion

Features are those of partially thrombosed basilar artery aneurysm, causing significant mass-effect on the brainstem (raising the possibility of neurovascular compression syndrome, needing clinical correlation and thin cut MRI study if indicated).

Hypoplastic right vertebral artery is also noted.

The patient also was found to have restricted diffusion in the right cerebellum (not shown) suggesting acute infarction of PICA territory.
