Presentation
Severe headaches.
Patient Data
There is a giant saccular aneurysm of the intracavernous segment of the left internal carotid artery measuring 35 x 30 x 25 mm with a narrowed neck measuring 2 mm. It shows two components, a central component attached to the posterolateral wall of high signal on T1, low signal on T2 and GE sequences, and peripheral component vividly enhanced following IV contrast. A mass effect is noted on the temporal lobe.

Case Discussion
MRI features of partially thrombosed giant cerebral aneurysm of the intracavernous internal carotid artery.
Giant cerebral aneurysms account for ~5% of all intracranial aneurysms and most commonly saccular form.
