Patella alta with Hoffa's fat pad impingement

High-riding dysplastic patella (patella to patellar tendon length = 1:1.6) with obvious signs of chondromalacia. Dysplastic medial trochlea with mal-alignment of the patella. Sub-synovial swelling and edema between the lateral femoral condyle and the lateral patellar ligament at the apex patellae.
