Patellar tendon-lateral femoral condyle friction syndrome

Case contributed by Dr Ammar Haouimi

Anterior pain of the left knee with a recent sports injury.

The MRI sequences demonstrate a focal area of high signal on PD fat sat (edema) installed the inferolateral aspect of the patellofemoral joint, within the superolateral portion of Hoffa's fat pad. The Install-Salvati ratio was at 1.45 (patella Alta).

The lateral meniscus shows a double anterior horn / flipped meniscus, indicating a bucket-handle tear. A small popliteal cyst is noted. No significant joint effusion is seen.

Case Discussion

MRI findings of a patellar tendon-lateral femoral condyle friction syndrome (also known as Hoffa's fat pad impingement syndrome) with associated patella Alta and bucket handle lateral meniscal tear.
