Axial image shows edema in Hoffa's fat pad between patellar tendon and lateral femoral condyle,
In addition, the Insall-Salvati index was 1.5 suggestive of patella alta.
Case Discussion
It is clinically known as Hoffa’s fat pad impingement syndrome. Patellar maltracking is considered to be the most frequent underlying cause of complaints in these patients.
Clinical suspicion of patellar tendon lateral femoral condyle impingement syndrome can be confirmed directly by imaging on MRI.
