Patent ductus arteriosus complicated by pulmonary arterial hypertension

Presentation

Tachypnea and respiratory distress.

Patient Data

Age: 12 months
Gender: Female

Large PDA is noted which measures approximately 8mm.

The pulmonary trunk along with right and left pulmonary arteries are dilated. The pulmonary trunk is measuring 20mm in diameter, the ascending aorta at this level measures 16mm.

The pulmonary veins are normally draining into the left atrium. Left heart chambers are dilated. Normal origin of coronary arteries is noted.

Consolidation involving posterior segment of right upper lobe, medial segment of middle lobe and posterior segment of left lower lobe.

Case Discussion

Imaging plays a central role in diagnosing and identifying the cause of pulmonary hypertension. CT features can be divided into vascular, cardiac and parenchymal findings. The comparison of pulmonary trunk to ascending aorta for diagnosis of pulmonary hypertension has a positive predictive value of 96%.
