Pelvi-ureteric junction obstruction with hydronephrosis (gross pathology)

Case contributed by Dr Andrew Ryan

PUJ obstruction on imaging

Patient Data

Age: 30 years
Gender: Male

Right nephrectomy specimen

The kidney has been bivalved to show a grossly dilated renal pelvis and calyceal system. While not visible in the picture, the ureter immediately distal to the PUJ measured 4mm in diameter.

Case Discussion

This is an macroscopic example of chronic PUJ obstruction with marked hydronephrosis. Histological examination of the pelvo-ureteric junction showed mild submucosal fibrosis and mild hypertrophy of the muscle wall but there was no evidence of malignancy.
