Presentation

Motor vehicle accident with severe pelvic pain and deformity.

Patient Data

Age: 5 years
Gender: Male
ct

Comminuted fractures of bilateral superior and inferior pubic rami.

Vertically oriented left sacral fracture extending along sacral ala traversing the sacral neural foramina.

Left iliac crest fracture extending to the left sacroiliac joint which appears disrupted and diastatic.

These findings are suggestive of combined mechanical pelvic fractures (lateral compression + vertical shear fractures).

Case Discussion

Pelvic fractures are common injuries after motor accidents.

Fractures of both superior and inferior pubic rami are considered unstable injuries. The mechanism assumes to be a vertical shear injury. It is associated with genitourinary tract injuries in 40% of cases.

Lateral compression fracture is a common pelvic fracture characterized by transverse fracture of pubic rami and ipsilateral/contralateral to posterior injury fracture, common mechanism is a rollover vehicle accident.
