Pelvic hydatid cyst

Diagnosis almost certain

Presentation

Vague lower abdominal pain. Positive hydatid serology.

Patient Data

Age: 65 years
Gender: Female
ultrasound

Transabdominal ultrasound revealed a large unilocular cystic lesion in the pelvis without internal echoes, septae or calcifications. Transvaginal ultrasound showed that the lesion is displacing the uterus posteriorly. 

Right lobe of liver shows a well-defined mixed echogenicity lesion consistent with inactive hydatid cyst (stage CE4 according to World Health Organization 2001 classification of hepatic hydatid cysts).

The spleen showed no lesions.

Note coexistent cholelithiasis.

Case Discussion

Cystic mass of ovarian origin and intraperitoneal hydatid cyst were kept as differentials. The lesion was surgically removed. Histopathology revealed hydatid cyst.
