Presentation

Traumatic intercourse.The patient presents with a painful, swollen and bruised penis.

Patient Data

Age: 40 years
Gender: Male

There is a focal defect and tear of the left tunica albuginea, ventral surface, and middle third of the penis. The defect measures 8 mm in length and 9 mm in width. A focal contusion of the left corpus cavernosum is present. Intact and normal right corpus cavernosum. Secondary multiseptated acute hematoma in the subjacent subcutaneous soft tissues was well identified on the coronal T2 images. The hematoma measured 57 mm in length with a width of 17.0 mm.

Intact and normal corpus spongiosum and urethra. Incidental bilateral small, simple, hydrocoeles.

Annotated images identify the site of the left-sided, ventral, tunica albuginea defect within the middle third of the penis. There is an adjacent multiseptated acute hematoma.

Case Discussion

A multiparametric MRI of the penis confirms a fracture of the left ventral tunica albuginea.

This is the MRI equivalent appearance of the clinical aubergine sign or egg-plant deformity of a markedly swollen, bruised and discoloured penis secondary to hemorrhage beyond the tunica albuginea at the tear site.

Based on the penile grading system developed by Shukla et al 1, this corresponds to a grade II.
