Perforated appendicitis

Presentation

One week history of right lower abdominal pain, fever and leukocytosis.

Patient Data

Age: 60 years
Gender: Male

The appendix is of retrocecal location, dilated (13 mm) with a thick enhanced wall, containing an appendicolith. Focal defect at the appendiceal tip with small adjacent air-fluid collection as well as periappendiceal fat stranding with thickening and enhancement of the peritoneal reflections. Reactive thickening with enhancement of the cecal wall and terminal ileum. 

Small left parapelvic cysts are noted.
