Presentation

Right-sided abdominal pain.

Age: 35 years
Gender: Male

The appendix shows diffuse thick wall with an appendicolith. It shows perifocal fat stranding and thickening of peritoneal reflections. Focal extraluminal air is noted at the level of appendicolith at its mid-segment flexure, denoting perforation.

Case Discussion

The patient was operated on the second day due to some medical contraindications to anesthesia. Operative finding was an appendiceal phlegmon.

The presence of extraluminal gas foci is a sign of localized perforation that can develop to periappendiceal phlegmon or abscess.
