Perforated duodenal ulcer with peritonitis

Presentation

Acute onset of abdominal pain.

Patient Data

Age: 25 years
Gender: Male

The CT scan demonstrates:

  • focal mural defect of the duodenal bulb (see annotated images) with adjacent fat stranding and small air-bubbles
  • free air within the peritoneal cavity mainly around the liver and spleen (pneumoperitoneum)
  • free fluid in the peritoneal cavity mainly in the pelvic region with mesenteric fat stranding and thickening of the peritoneal reflections
  • paralytic ileus
