Perforated emphysematous cholecystitis with abscess formation

Presentation

Mild right upper abdominal pain with fever in a diabetic patient.

Patient Data

Age: 65 years
Gender: Female

The gallbladder is relatively distended with thick enhanced wall and intramural and intraluminal gas. Two gallbladder wall defects are noted, towards segment 4 with extraluminal gas and segment 6 with abscess formation. Fat stranding around the gallbladder.

Other findings:

  • small hiatal hernia

  • left renal stones
