Perforated gallbladder with subcapsular biloma

Severe right upper quadrant pain for 6 days.

Patient Data

Age: 75
Gender: Female

Distended inflamed gall bladder with a central gas-filled calculus impacted in the neck. Surrounding inflammation in the adjacent fat. Defect in the gall bladder fundus with large fluid collection around the liver indicates perforation. Note that the fluid collection conforms to the liver contour indenting the parenchyma with a scalloped appearance. This indicates a subcapsular collection of bile.
