Free peritoneal gas is visualized predominantly in the upper abdomen. The perforation site is seen in the pyloric region / pyloric canal of the stomach. A small locoregional hyperdensity resembles a blood clot (compare contrast and non-contrast series).
There are moderate collections of free intraperitoneal fluid.
Liver, renal, and splenic cysts are noted.
Case Discussion
This case shows a perforated peptic ulcer of the stomach. The perforation site is visible on CT, located in the prepyloric region (Type III in the modified Johnson classification).
