Herniation of a large collection of small bowel loops (ileum) through a caecal mesenteric defect, behind and lateral to the caecum, and into the right paracolic gutter.

The herniated bowel is swollen and the mesentery is oedematous with free intraperitoneal fluid = ischaemic (at least venous).
