Pericardial cyst

Presentation

Cough for 2 months with greenish phlegm, short of breath, waking up at night, ?chest infection

Patient Data

Age: 45 years
Gender: Female
x-ray

No focal lung lesion, collapse or consolidation.

Abnormal shape/contour at the right heart border, consistent with known right-sided pericardial cyst (first identified on CT Thorax below).

7 years prior

x-ray

No focal lung lesion, collapse or consolidation.

Normal heart size and mediastinal contour.

CT was performed for query cause of hemoptysis.

There is a 6 cm ovoid cyst abutting the right atrium with Hounsfield value of around 30 units which is most likely to be a pericardial cyst.

This would not account for symptoms of hemoptysis.

No malignancy demonstrated.

mri

A pericardial cyst is seen adjacent to the right atrium. It is crescentic in shape measuring 5.2 x 4.6 x 2.1cm. It doesn't exert any effect on the adjacent cardiac chambers. It appears to be a simple cyst.

Case Discussion

Pericardial cysts are typically picked up incidentally, as in this instance. Asymptomatic patients are usually treated conservatively.

Serial transthoracic echocardiograms may be conducted to assess for stability, ensuring the cyst is not enlarging 1.
