Cough and hypoxia.

Patient Data

Age: 65
Gender: Male

Cardiac silhouette is enlarged. Basal interstitial thickening, which may reflect lower respiratory tract infection, inflammation or interstitial edema. No pleural effusion.

No filling defect demonstrated within the pulmonary arteries. Pericardial effusion (simple fluid density), measuring up to 21 mm in depth.

This case demonstrates that not all enlarged cardiac silhouettes on chest x-ray are due to cardiomegaly.
