Pericardial effusion

Presentation

Known hypertensive with Sjogren syndrome and hypothyroidism. The patient presents with a chronic cough, diarrhea, vomiting, and stomach pain. She has a fractured right leg.

Patient Data

Age: 60 years
Gender: Female
ultrasound

Moderate circumferential pericardial effusion measuring 1.4cm in diastole . Mild bilateral pleural effusion left more than right. There is right ventricular outflow tract diastolic collapse and right atrial systolic collapse. No abnormal respiratory variations of transmitral flow. Normal inferior vena cava with more than 50% inspiratory collapse. Normal left ventricular systolic function.

Case Discussion

This is a case of moderate pericardial effusion with impending tamponade. There was no pulsus paradoxus and no ventricular coupling (interdependence).
