Presentation

Perineal swelling for 1 week, with pain on sitting

Patient Data

Age: 35 years
Gender: Female

A well-defined cystic lesion is noted at the perineal region with regular wall and mild turbid fluid content with low-level echoes. It is occupying The perineum encroaching upon the lower third of vagina. No fistulous collections or other perineal cystic lesions identified. Its lower part lies just under the skin. 

Case Discussion

This is a pregnant woman in her second trimester that complained of perineal swelling and pain on sitting. A midline perineal cystic lesion with no lateralization excludes the possibility of Bartholin gland origin (cyst or abscess). The lesion reaches perineal skin with turbid fluid content, which revealed to be a perineal abscess on drainage.

Transperineal ultrasound is an easy ultrasound technique used in uro-gynecologic and ano-rectal evaluation with various applications as in pelvic floor dysfunction, inflammatory lesions, perianal fistula, US-guided procedures, and assessment of sphincters.
