Periorbital cellulitis with subperiosteal collection and intracranial extension

Presentation

Headache and fevers for several days. Now eye swelling and redness with raised inflammatory markers and nasal discharge.

Patient Data

Age: 12
Gender: Male

Extensive inflammatory change of the paranasal sinuses with a left sided periorbital cellulitis, orbital roof subperiosteal collection and associated breakthrough into the intracranial cavity with an extradural abscess.

Case Discussion

Periorbital cellulitis needs to be investigated thoroughly to identify any associated complications. As with any imaging, finding one complication (the extradural abscess) should not stop the search for other complications - the subperiosteal collection. Both need drained and covered with appropriate antibiotics.
