Perirenal transplant hematoma

Presentation

Falling Hb D2 post renal transplant.

Patient Data

Age: 50 years
Gender: Male

Large hematoma lies anteriorly to the right iliac fossa transplant kidney and extends into the pelvis. RIF transplant kidney JJ stent and drain tube noted. Post-surgical changes are noted, along with a small right rectus sheath hematoma. Small volume of intraperitoneal free fluid.

Left iliac fossa transplant kidney with a cyst at the upper pole. Innumerable cysts on both native kidneys. Small bilateral pleural effusions with associated lower lobe atelectasis.

Case Discussion

Small perirenal transplant hematomas are common but larger hematomas less so. Often their size is underestimated on ultrasound.
