Presentation

Pelvic pain. History of right adnexectomy for an ovarian cyst (borderline mucinous cystadenoma).

Patient Data

Age: 40 years
Gender: Female

A large multilobulated peritoneal cystic structure with thin septa, surrounding the left ovary of homogeneous fluid content of low signal on T1, high signal on T2 with no restricted diffusion. Mild peripheral enhancement is seen on postcontrast sequences with no solid components.

MRI features most consistent with a peritoneal inclusion cyst in a patient with a history of right adnexectomy for borderline mucinous cystadenoma.

Peritoneal inclusion cyst is defined as a cyst-like structure that appears in relation to the peritoneal surfaces, caused by the accumulation of ovarian fluid that is contained by a peritoneal adhesion. It occurs almost exclusively in premenopausal women with a history of previous abdominal or pelvic surgery, trauma, pelvic inflammatory disease, or endometriosis.
