Presentation

Throat pain for the past 3 days. Complains of "scratchy" voice.

There is asymmetric thickening of the left oropharyngeal soft tissues with a peritonsillar mass, which is centrally hypoattenuating with peripheral enhancement. This mass results in moderate mass effect on the upper airway, occupying at least 50% of the oropharyngeal diameter.

Associated inflammatory changes and edema tracks inferiorly down the parapharyngeal mucosal space, narrowing the upper pharyngeal airway and causing asymmetry of the hypopharynx and effacement of the left piriform sinus.

Case Discussion

This is a case highly suggestive of a peritonsillar abscess.

Following the CT, the patient left against medical advice before he could be seen by otolaryngology for an attempted aspiration/drainage.
