Glioblastoma NOS (periventricular)

Diagnosis certain

Irregular enhancing mass of the left splenium, left parietal lobe and left medial temporal lobe with surrounding vasogenic edema.

Case Discussion

The patient went on to have a biopsy. 

Histology

Microscopic Description:

Sections show pieces of a high-grade glioma displaying astrocytic characteristics predominantly. Areas of necrosis, including geographic areas of necrosis with nuclear pseudopalisading, are seen. Focal endothelial cell proliferation is observed. Some vessels display thrombotic occlusion of the lumen. Nuclear pleomorphism is increased. The tumor cells display, predominantly, astrocytic features. Mitotic figures are identified without difficulty.

FINAL DIAGNOSIS: glioblastoma

Note: IDH mutation status is not provided in this case and according to the current (2016) WHO classification of CNS tumors, this tumor would, therefore, be designated as a glioblastoma NOS
