Presentation

Pain at the external malleolus for several weeks.

Patient Data

Age: 45 years
Gender: Male

Partial longitudinal split tear of the peroneus brevis tendon with 'boomerang sign' on ultrasound.

Case Discussion

Peroneus brevis tendon tears can present with lateral ankle pain with or without a history of trauma, worsening with activity. Torn peroneus brevis tendons have a characteristic C-shape or "boomerang" appearance, with the tendon enveloping the peroneus longus tendon.
