A bifid median nerve is an uncommon anatomical variation in the forearm, it can be accompanied by a persistent median artery

This anatomical variant should be mentioned in the ultrasound report to avoid inadvertent injury to the nerve and artery during the surgical release of the transverse carpal ligament.
