Perthes lesion

Presentation

Multiple shoulder dislocations.

Patient Data

Age: 15 years
Gender: Male
mri

Tear of anteroinferior labrum extending superiorly. Mild loss of adjacent anteroinferior glenoid cartilage is also seen. Adjacent periosteum is mildly lifted from the anterior aspect of glenoid; however, appears intact. No obvious bony loss is seen in the present scan. The tear is possibly extending from 5 to 12 o’clock position.

Posterior labrum appears normal. 

Hill Sach’s compression fracture with underlying marrow edema seen on posterolateral aspect of head of humerus. It measures approx. 15mm in transverse dimension and 4.5mm in depth.

Accessory ossicle is seen adjacent to acromion process consistent with os acromiale.

Partial interstitial tear of posterior fibers of supraspinatus tendon seen involving approx. 20-30% thickness. 

Case Discussion

Hill Sach lesion with a large labral tear. Mildly elevated adjacent periosteum, which is however intact, consistent with Perthes lesion.
