Presentation

Ankle stiffness and reduced range of motion.

Patient Data

Age: 45 years
Gender: Male

Weight-bearing

x-ray

Pes cavus alignment (increased Meary’s angle, normal calcaneal pitch) with sinus tarsi "see-through sign" as well as hindfoot valgus with increased talocalcaneal angle.

Large ossicle with irregular sclerotic margins - os vesalianum vs non-united avulsion fracture.

Case Discussion

This is an unusual alignment, which was asymmetric to the left foot (not shown). The underlying cause is not known.
