Presentation

Pain in the right 3rd digit.

Patient Data

Age: 25 years
Gender: Male
x-ray
  • well-marginated diaphyseal-metaphyseal unique lucent lesion involving P1 of the right 3rd digit

  • associated pathological fracture within this lesion

  • small rings and arcs calcifications within the lesion matrix

  • no cortical breach, gross bone destruction, or soft tissue extension

  • no periosteal reaction

mri
  • sharply-defined diaphyseal-metaphyseal unique lesion involving P1 of the right 3rd digit. It demonstrates a chondroid matrix -T1 isointense (compared to muscle), and T2 hyperintense

  • enhancement of the scalloped margins with curvilinear septa (rings and arcs enhancement pattern)

  • the fracture is not seen on T1 sequences

  • no soft tissue involvement

Case Discussion

This case demonstrates features of an enchondroma that may also be seen in low-grade chondrosarcoma, as the lesions are both histologically and radiographically very similar.
