Presentation
Hoarseness of voice for 6 days, endoscope revealed a foreign body in the left supra-glottic region prepared for endoscopy and removal.

A bi-limbed V-shaped hyperdense foreign body (FB) structure is seen at the laryngopharynx & to the left side of the supra-glottic region. It is likely to represent a metallic (opened) safety pin. Its upper transverse limb passing anteriorly penetrates the anterior laryngeal wall below the level of the epiglottis. It penetrates the left pyriform sinus through the aryepiglottic fold so, its lower limb is directed downward through the trans-glottic space indenting the left vocal cord till teaching the sub-glottic space and lower larynx.

The opened safety pin was found impacted as described in the above findings.
Case Discussion
Pharyngolaryngeal foreign body aspiration is an uncommon but potentially life-threatening event. Plain XR and computed tomography can provide detailed information regarding the accurate location and nature of the foreign body. The endoscopic procedure remains the gold standard for diagnosis and management.
