Physiological neonatal breast enlargement

Presentation

Swelling of both breasts. No fever or localized redness.

Patient Data

Age: 20 days
Gender: Female

Bilateral and relatively symmetrical heterogeneously hypoechoic lesions measuring (33 x 13 mm) on the right and (25 x 12 mm) on the left, containing scattered tiny cysts surrounding subcutaneous soft tissues.

Case Discussion

Neonatal breast enlargement is a relatively common condition that can occur in both female and male neonates due to the influence of maternal estrogens during fetal life. It can be unilateral or bilateral (frequently bilateral), asymptomatic, or physiological. It typically appears within the first week of life and resolves spontaneously within the first six months.

This process differs from benign premature thelarche, which represents isolated breast development in females aged six months to nine years due to subtle overactivity of the pituitary-ovarian axis. If no other signs of puberty are present, reassure the family that this is a benign finding.
