Coronal non-contrast

Subtle low density is seen involving the medial and inferior aspect of the left cerebellar hemisphere, suggestive of an early PICA territory infarct.
Axial DWI

Acute restricted diffusion is demonstrated in the left PICA territory in keeping with an infarct. There is only minor swelling, no evidence of effacement of the fourth ventricle and no hydrocephalus. No other regions of restricted diffusion.
Case Discussion
Typical appearances of a PICA territory infarct hi-lighting how subtle early infarction can be in the posterior fossa.
