Fractures of the medial and lateral malleoli with considerable displacement and angular deformity of the lateral malleolar fragment. The fracture is through the tibial plafond, making this a pilon fracture.

Case Discussion
Pilon fractures are fractures of the distal tibial plafond.
This patient had marked comminution as well as articular impaction of the distal tibial plafond. According to the Ruedi and Allgower classification, this is a type III Pilon fracture.
