Pineal cyst

Presentation

Chronic headache. Incidental finding.

Patient Data

Age: 30 years
Gender: Female
mri

Well-circumscribed ovoid cystic lesion of the pineal region measuring 12 x 10 x 8 mm, eliciting a low signal on T1, high signal on T2,  not attenuated on FLAIR and with no restricted diffusion. The postcontrast sequences show a thin peripheral enhancing rim with no solid component.

MRI features most consistent with a pineal cyst.

Pineal cysts are common, usually small (<1 cm) asymptomatic, and typically found incidentally in young adults (20-30 years of age) with a predilection for women. When larger they can present with compression of the aqueduct with obstructive hydrocephalus.
