Pineal cyst

Presentation

Motor vehicle collision. Evaluate for intracranial hemorrhage.

Patient Data

Age: 17 years
Gender: Male

There is a hypodense cystic lesion in the pineal gland with a small focus of calcium anteriorly.

No evidence for acute intracranial abnormality. Ventricles are normal in size, shape and position. Mild cerebellar tonsillar ectopia is noted.

Follow-up MRI

There are multiple T2 hyperintense cysts within the pineal gland with the largest lesion measuring up to 9 mm. There is no associated nodular or masslike enhancement. There is no evidence of a significant associated mass effect on the tectum.

Case Discussion

This is highly suspected case of a pineal cyst. This was an incidental finding on the initial CT. Given the size, an MRI was ordered which further suggested a pineal cyst. The patient was scheduled with neurosurgery who opted for nonsurgical observation.
