Pineal epidermoid cyst

A small rounded area of high T2, low T1 without enhancement is present in the pineal region that has very high DWI signal and ADC values similar to the brain. This is characteristic and essentially pathognomonic of an epidermoid cyst.
Case Discussion
The signal characteristics of epidermoid cysts are very characteristic and essentially pathognomonic, especially when located in an extra-axial location.
